Back when my Logitech wireless KBM worked properly (mouse now dead, KB dying) it worked fine in D2x. I haven't played much since I started using the Intellimouse Explorer, but I think it worked fine last time I did. Maybe you should try mapping keys in D2, as it lets you map mouse buttons too.

Is there any way to make Diablo 2 always open in window mode?

If i quit the game whilst in window mode the next time i play its back to full screen again. :/
